Russia has changed the form of the certificate of vaccination against COVID-19

In RUSSIA, the order of the Ministry of HEALTH on amending the form of the certificate of vaccination against CORONAVIRUS came into force. The document was published on the official Internet portal of legal information.

The new form of the certificate was approved by the Ministry of Health on October 25. The document will contain the patient's personal data (name, date of birth, gender, address, passport details, SNILS, medical policy), vaccination information (date, name of the drug and the organization where the injection was given), medical contraindications (if any) and about the transferred covid-19 , if the bearer was still ill with it. In addition, a QR code will fit on the first page in the upper corner.

The certificate will be issued in Russian and English, in electronic (on the "State Services" no later than three days after vaccination) and paper forms.

If the certificate was received before November 8, then it will be automatically reissued at the State Services until March 1, 2022.
