The Ministry of HEALTH approved the form of a certificate of vaccination against covid-19, the order is published on the official Internet portal of legal information.

The order comes into force on November 8. Certificates that Russians received before this date will be automatically reissued on the Gosuslugi portal until March 1, 2022.

The approved form of the certificate fit on two pages. Its items include:

patient's personal data; vaccination data - date, which drug was used, where the injection was given; information about medical contraindications, if any; information about the transferred COVID-19, if the person still had the infection.
